About the Role

Somerset County Council is seeking a Health Visitor Team Manager to join our team. The successful candidate will be responsible for ensuring the co-ordination and delivery of skilled, effective care to promote positive health and respond to identified, individual client and population/community needs.

\

The ideal candidate will have a strong background in public health, with experience in tackling disadvantage and inequality to impact on the health divide. They will also have excellent communication and leadership skills, with the ability to work across agencies and manage a caseload.

\

The salary for this role is £42,182 - £50,677 per annum, depending on experience. Benefits include flexible working patterns, prioritized supervision, and opportunities for continuing professional development.

Key Responsibilities:\

  • Ensure the co-ordination and delivery of skilled, effective care.\
  • Develop and implement programs of intervention based on identified individual health and social needs.\
  • Work in partnership with other agencies, particularly health services, education, and social care.\
  • Maintain a working knowledge of public health targets and local delivery plans.\
  • Develop and deliver programs of education and support to targeted individuals and groups within the community.\
  • Prescribe from the Nurse Prescribers Formulary for District Nurses and Health Visitors.\
  • Provide safe and competent advice for individual clients receiving care.\
  • Offer advice, guidance, and support to children and families.\
  • Understand, uphold, and promote the aims of the councils equality, diversity, and inclusion policies.\
  • Communicate in person, face-to-face, in writing, and electronically with all contacts.\
