Livewell Southwest is an independent, award-winning social enterprise providing integrated health & social care services for people across Plymouth, South Hams & West Devon, as well as some specialist services for people living in parts of Devon & Cornwall.
We are seeking a skilled Family Health Worker to join our team. This is an exciting opportunity to make a real difference in the lives of families and children in our community.
The successful candidate will work within a Public Health Locality Team, providing support to families with children aged up to 5 years. You will be responsible for delivering the DOH Healthy Child Program 0-5years and implementing approaches to promote and help prevent adverse effects on health and well-being.
This role is ideal for someone who is passionate about working with children and families, and has a strong commitment to public health. You will have excellent communication and interpersonal skills, and be able to work effectively in a team.
Main Duties
- To plan and undertake home visits to deliver an episode of care, as delegated and assessed by the SCPHN in response to identified need.
- To work in partnership with families, health professionals and other agencies as required.
- To communicate and liaise clearly and effectively with clients, colleagues and a variety of agencies within community settings and base.

Person Specification
Essential
- Able to communicate effectively
- Able to work alone within a supervised framework
- Decision maker
- Enthusiasm for working with children and families
- Good listening and observational skills
Desirable
- Works on an individual basis with children and families, supervised by the SCPHN, for time limited, outcome based episodes of care
- Is able to recognise areas of concern within families and follow agreed procedures
- Creative
- Awareness of importance of play for children
- Positive attitude
Qualifications
Essential
- Vocational Qualification Level III in childcare or health related area
- Maths & English GCSE Grades 4 and above or equivalent
- Additional qualifications in child and family related areas, for example play skills, behaviour management, Portage, budgeting, nutrition or on the job training
Desirable
- First aid training
- Knowledge of community initiatives
- Knowledge of child protection
- Knowledge and/or experience of developmental language delay
- Experience with group work
