Advanced Nurse Practitioner

We are seeking an experienced Clinical Nurse Lead to join our team at Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust. As a Clinical Nurse Lead, you will play a key role in developing and implementing high-quality patient care plans, working collaboratively with multidisciplinary teams to achieve excellent patient outcomes.
About the RoleThe successful candidate will have a strong background in nursing, with experience in leading clinical teams and developing care plans. You will be responsible for participating in collaborative working with patients, carers, and the multidisciplinary team to facilitate high standards of individualized patient care and progression to successful discharges.
Main Responsibilities- Develop and implement high-quality patient care plans
- Lead clinical teams to achieve excellent patient outcomes
- Participate in collaborative working with patients, carers, and the multidisciplinary team
- Deliver clinical supervision, appraisals, and training opportunities to support staff in achieving high standards of care

RequirementsTo be successful in this role, you will need to have:
12 to 24 months band 5 qualified nurse experience dependent upon service needA strong background in nursing and experience in leading clinical teamsExcellent communication and leadership skillsThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such, it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
