Job Opportunity: We are seeking a highly skilled and experienced Practice Nurse to join our team at Menlo Park Recruitment. This is a fantastic opportunity for a passionate and dedicated individual to contribute to a multidisciplinary team renowned for excellence in patient-centred care.

Key Responsibilities:

  • Provide high-quality nursing care to patients with chronic diseases, such as diabetes and COPD.
  • Work collaboratively with a multidisciplinary team to deliver innovative and effective patient care.
  • Contribute to the development and implementation of nursing policies and procedures.
  • Participate in clinical meetings and contribute to the improvement of patient outcomes.
  • Provide education and support to patients and their families on chronic disease management.

Requirements:

  • Essential experience as a Practice Nurse comfortable with Chronic Disease Management.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility around start and finish times.
