Learning Disability Support Professional

Join us as a Senior Practitioner in our Social Care team We are looking for an experienced Adult Social Worker to provide support services for adults with learning disabilities, conduct assessments, and develop quality systems across the city.
Key Responsibilities:- Conduct home visits and assessments to identify individual needs and create personalized care plans
- Develop and maintain effective relationships with clients, families, and colleagues to ensure high-quality service delivery
- Provide expert advice and guidance to social care staff on complex case management issues
- Supervise junior staff members and contribute to their professional development
- Stay up-to-date with relevant legislation, policies, and best practices to deliver exceptional care
Competitive salary: £40,000 - £50,000 per year.
