Job Summary

We are seeking a highly skilled and dedicated Nurse Practitioner to join our team at Holmhurst Medical Centre. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and contributing to the success of our medical centre.

Main Responsibilities

  • Administer adult and child immunisations and vaccinations in accordance with national and local programmes, and ensure safe storage, rotation and disposal of vaccines and drugs.
  • Undertake procedures to include venepuncture, basic wound care, BP management, urinalysis, peak flow, spirometry, stitch/clip removal and cervical smears.
  • Work within all relevant practice policies and procedural guidelines, including infection control, chaperoning and patient group directions.
  • Initiate the process of diagnosis with patients suspected to have a chronic disease, such as diabetes, COPD, asthma, CHD, and refer to other clinicians as appropriate.
  • Manage programmes of care for patients with chronic diseases by planning, providing and evaluating care under agreed local/national guidelines, protocols and patient group directions.
  • Contribute to a satisfactory call and recall system of patients and undertake annual review of patients.
  • Work collaboratively as a team member to achieve the objectives of the NHS contract and aspirations of the team.
  • Provide expert clinical advice on educational opportunities that will facilitate the development and support of their specialist knowledge and skills.
  • Recognise the development needs of others and provide clinical supervision.
  • Develop own knowledge and practice to meet objectives/changes in service.
  • Make effective use of the clinical system by ensuring the consistent and accurate entering of data and use of SNOMED Codes.

Holmhurst Medical Centre is a member of a Primary Care Network (PCN) and Dorking Healthcare Federation. We are a 3-partner General Medical Services (GMS) practice with an increasing list size (currently 10,200 patients). We are a training practice and have been rated Good by the Care Quality Commission (CQC). We consistently achieve high Quality and Outcomes Framework (QoF) targets and have a multidisciplinary team committed to our local community.
