About the Job:

We are recruiting for Block Booking Community Care Professionals to work in the West Midlands Area. As a key member of the District Nursing team, you will play a vital role in providing comprehensive care to patients in their own homes.

Job Description:

  • Undertake responsibility for caseloads, assessments, planning, and evaluation of patients' care
  • Actively support the District Nursing team in delivering excellent patient outcomes
  • Work collaboratively with other healthcare professionals to ensure comprehensive care
