Paediatric Nurse Case Manager RoleAbout the Role:

This is an exciting opportunity to work as a Paediatric Nurse Case Manager in a dynamic healthcare environment. You will be responsible for providing high-quality patient care and working closely with other healthcare professionals to achieve excellent outcomes.

Responsibilities and Accountabilities:
  • To design, deliver, and evaluate tailored care plans for clients with complex needs.
  • To lead and manage a multidisciplinary team, promoting effective communication and collaboration.
  • To provide comprehensive assessments, interventions, and support to meet the physical and emotional needs of clients.
  • To build and maintain relationships with clients, families, and other stakeholders.
  • To contribute to the development of policies and procedures, ensuring adherence to regulatory requirements.
Requirements:
  • RSCN registration with the NMC and a minimum of 6 months' experience in the UK.
  • A supportive management style, with the ability to motivate and inspire others.
  • Up-to-date knowledge of tracheostomy care and ventilator management.
  • Excellent communication and interpersonal skills.
  • Suitable for Band 5/Band 6 Nurses with evidence of leadership and management experience.
  • Desirable: Full UK driving licence.
