Care Coordinator Job Description

We are seeking a skilled and empathetic Care Coordinator to provide high-quality care and support to elderly patients at our healthcare facilities.

As a key member of our care team, you will oversee the day-to-day care activities and ensure that our residents receive the best possible care.

You will have a strong understanding of clinical care for the elderly and be able to develop and implement individualised care plans that meet the unique needs of each resident.

Key Responsibilities:

  • Supervise and motivate a team of Care Assistants
  • Work closely with our clinical team to deliver seamless care
  • Develop and implement individualised care plans

Salary: £24,000 - £28,000 per year.
