We are seeking an experienced and compassionate Social Worker to join our Older Persons Services Team in Wrexham Council. As a Social Worker, you will be responsible for providing high-quality care and support to elderly individuals, promoting their independence and well-being.

Key Responsibilities:

  • Assessing the needs of elderly individuals and developing personalized care plans
  • Providing emotional support and guidance to individuals and their families
  • Collaborating with other healthcare professionals to ensure comprehensive care
  • Monitoring and evaluating the effectiveness of care plans

Requirements:

  • Qualification in Social Work or a related field
