We are seeking an experienced and compassionate Social Worker to join our Older Persons Services Team in Wrexham. As a Social Worker, you will be responsible for providing high-quality care and support to elderly individuals, ensuring their well-being and dignity are maintained.
Key Responsibilities:- Assess and identify the needs of elderly individuals, developing and implementing personalized care plans.
- Provide emotional support and guidance to individuals and their families, promoting independence and self-advocacy.
- Collaborate with healthcare professionals and other stakeholders to ensure seamless care delivery.
- Conduct home visits and assessments, gathering information to inform care decisions.
- Develop and maintain relationships with community resources and services to enhance care options.
- Qualifications in Social Work or a related field.
- Experience working with elderly individuals and their families.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Flexibility to work varied hours, including evenings and weekends.
