Children's Community Continuing Health Care Staff Nurse

We are seeking a dedicated and compassionate Registered Nurse to join our Children's Continuing Care Team. As a key member of our team, you will be responsible for supporting the holistic assessment of children's care needs and developing, implementing, and evaluating programmes of care within a Continuing healthcare caseload.
The ideal candidate will have a strong understanding of the needs of children and young people with complex health needs and be able to work effectively with families and other healthcare professionals to provide high-quality care.
As a member of our team, you will have the opportunity to make a meaningful impact in the lives of children and their families, and contribute to the development of our Continuing Care Service.
Key Responsibilities:
- Support the holistic assessment of children's care needs
- Develop, implement, and evaluate programmes of care within a Continuing healthcare caseload
- Work effectively with families and other healthcare professionals to provide high-quality care
- Contribute to the development of our Continuing Care Service
Requirements:
- Current NMC PIN to practice as a Registered Nurse or Midwife in the UK
- Ability to work effectively with children and young people with complex health needs
- Strong communication and interpersonal skills
- Ability to work as part of a team
