Job Title: Clinical Pharmacist

We are seeking a highly motivated and experienced Clinical Pharmacist to join our team at Grovelands Medical Centre. As a Clinical Pharmacist, you will play a pivotal role in delivering our pharmacy vision and providing excellent pharmacy-led services for patients registered with our practice.

Main Responsibilities:
  • Contribute to the leadership of medicines-related issues for the surgery and provide leadership and mentoring to clinical pharmacists, pharmacy technicians, and prescribing support staff.
  • Maintain up-to-date clinical knowledge and a thorough understanding of primary care prescribing, medication reviews, and strategies for improving prescribing.
  • Uphold the NHS commitment to addressing health inequalities and ensure care is available to people from all communities.
  • Manage the repeat prescription system, acute prescription requests, and medicines on transfer of care.
  • Provide telephone clinics for patients with questions about their medicines.
  • Perform structured medication reviews and manage aspects of the quality and outcomes framework and enhanced services.
  • Develop relationships between local community pharmacies and the PCN pharmacy team.
  • Deliver medicines-related training for GP and ACP prescribers in the PCN.
  • Conduct periodic meetings for reviewing prescribing and patient safety issues.
  • Lead the Prescribing Quality Scheme, PINCER, and QoF aspects for prescribing and audits.
  • Work with care home staff to improve the safety of medicines ordering and administration.

This role is crucial to improving the quality of care and operational efficiencies, requiring motivation and passion to deliver excellent service within general practice.
