Job Title: Clinical Pharmacist

We are seeking a highly skilled and experienced Clinical Pharmacist to join our Primary Care Network team at Wargrave Surgery in Berkshire.

About the Role

This is an exciting opportunity for a Clinical Pharmacist to work in a patient-facing role, clinically assessing and treating patients using their expert knowledge of medicines for specific disease areas. The successful candidate will take responsibility for areas of chronic disease management, undertake clinical medication reviews to proactively manage patients with complex polypharmacy, and oversee our dispensary, including policies and audits.

Key Responsibilities
  1. Develop and manage a medicines management plan and deliver patient services as determined by Network policy and local and national guidance.
  2. Maximise cost-effective prescribing and improve the quality of patient care.
  3. Assist in the writing and implementing of policies for our dispensary and overseeing audits and targets for the dispensary.
  4. Provide expertise in clinical medicines review and address social needs of patients in GP practices.
  5. Annual review of Dispensary policies in accordance with national guidance.
  6. Annual oversight and reviews of dispensary DRUM Audit.
  7. Reduce inappropriate polypharmacy and wasteful prescribing through clinical medication review.
  8. Reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure patients receive the medicines they need post-discharge.
  9. Contribute to reductions in medicine-related hospital admissions and readmissions by supporting patients to get the best outcomes from their medicines and identifying and addressing medicines-related issues.
  10. Manage the repeat prescribing re-authorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates.
  11. Manage patients and health care professionals' medicine queries.
  12. Increase quality and safety of prescribing through mechanisms such as audit and PDSA cycles.
  13. Support achievement of PQS and QOF targets.
  14. Implement drug withdrawals and alerts e.g. MHRA aimed at improving medicines safety.
  15. Work with primary care professionals and patients to implement NICE and other evidence-based guidelines.
  16. Run clinics where medicines are the main interventions e.g. anticoagulant monitoring clinics.
  17. Provide medicines information and training to Practice healthcare professionals and admin staff.
  18. Act as a source of medicines information for all of the practice team and patients (e.g. around doses, side effects, adverse events, possible alternatives e.g. around out of stocks).
Person Specification
  • Thorough and pay attention to detail.
  • Work well under pressure.
  • Sensitivity and understanding.
  • Patience and the ability to remain calm in stressful situations.
  • Maths knowledge and skills.
  • Experience of working in primary care.
  • Experience of working in a GP practice.
Qualifications
  • Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council.
  • Minimum of 2 years post-graduate experience in pharmacy, as demonstrated within a practice portfolio.
  • Completion of the CPPE pathway.
  • Member of the Royal Pharmaceutical Society (RPS).
  • Member of or working towards (through foundation) RPS Faculty membership.
  • Has an awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice.
  • Complete appropriate Continuing Professional Education and Development and remain in good standing with pharmacist registration and regulation bodies.
  • Maintain appropriate professional insurance.
  • May hold or be working towards a prescribing qualification.
  • May hold or be working towards a postgraduate pharmacy qualification.
