Job Title: Team Leader Part Time

Job Summary:

We are seeking a highly skilled and experienced Team Leader to join our team at The Forward Trust. As a Team Leader, you will be responsible for leading a team of Health and Wellbeing Practitioners, providing supervision, training, and guidance to practitioners, and ensuring the effective management and monitoring of services.

Key Responsibilities:

  1. Lead a team of Health and Wellbeing Practitioners, contributing to targets set by Forward and local commissioners.
  2. Provide supervision, training, and guidance to practitioners and support the management team.
  3. Alert the Service Manager to any significant risks or problems arising in the management and monitoring of services.
  4. Ensure case management/programme documentation is maintained to expected audit standards.
  5. Ensure that case management interventions are delivered according to the needs of service users and are CQC compliant.

Requirements:

  1. Experience of working within substance misuse services in a prison.
  2. Direct line management experience, providing supervision, appraisals, guidance, and constructive feedback.
