This is an exciting opportunity to join our team as a Mental Health Care Home Liaison Practitioner. As a Band 6 Care Liaison Nurse, you will work closely with healthcare professionals to deliver high-quality interventions to individuals residing in care homes in the Wiltshire area.
With significant skills and experience in educating and training, you will be responsible for developing and delivering education programs to non-specialist staff to ensure that they have the skills and knowledge required to support individuals with dementia and/or functional mental health conditions.
The ideal candidate will have a detailed understanding of the relevant legal frameworks/legislation and be able to apply this knowledge in a practical setting.
The estimated salary for this role is £38,000 - £50,000 per annum, depending on experience.
