Job Description:

This position involves providing specialist nursing care to patients in their own homes or local clinics, focusing on promoting independence and achieving healthcare goals.

You'll work collaboratively with other healthcare professionals to develop and implement tailored care plans, ensuring patients receive the best possible support.

Responsibilities:
  • Deliver expert nursing care, including assessments, interventions, and education to promote optimal health outcomes.
  • Conduct thorough assessments to identify patient needs, developing personalized care plans accordingly.
  • Work closely with other healthcare professionals to ensure seamless care delivery and coordination.
Requirements:
  • Degree in Nursing or equivalent qualification.
  • Registered with the NMC as a Registered Nurse (Adult).
  • At least 1 year of experience working in community settings, preferably in adult care.
Salary and Benefits:

This is a challenging yet rewarding role, with a salary of £35,000-£45,000 per annum, dependent on experience.
