Job SummaryWe are seeking a highly organized and detail-oriented individual to join our team as an Apprentice Care Navigator. As a key member of our support staff, you will play a vital role in ensuring the smooth operation of our medical centre.
Key Responsibilities- Prepare consulting rooms with clinical stationery, test requests, and sample bottles, following established protocols and checklists.
- Close down consultation rooms, secure the building, and perform daily tasks as required.
- Check pharmacy fridge temperatures, record temperatures, and take necessary actions to maintain a safe and compliant environment.
- Conduct weekly and monthly building checks, identifying and reporting any issues or concerns.
- Assist with fire safety procedures, including fire drills and emergency response.
- Reconcile evening appointments, ensuring accurate and timely recording of patient information.
- Provide exceptional patient care, navigating patients to the most suitable healthcare professionals and ensuring appointments are allocated as required.
- Communicate effectively with patients, staff, and external partners, maintaining confidentiality and adhering to data protection policies.
- Process new patient registrations, promote digital services, and contribute to quality outcomes initiatives.
- Book appointments with clinical teams, ensuring patients receive the correct care and attention.
- Deal with home visit requests, emergency presentations, and other patient-related matters.
- Maintain accurate and up-to-date patient records, ensuring correct information is recorded and available for clinical teams.
- Participate in practice uptake encouragement initiatives, contributing to quality outcomes and framework requirements.
- Receive visitors and deliveries, adhering to established protocols and procedures.
- Ensure staff sign-in and sign-out procedures are followed, maintaining accurate records and handover diaries.
- Adhere to safeguarding and child protection policies, reporting any concerns or issues promptly.
- Scan, code, and file patient information accurately and efficiently.
- Perform electronic document management tasks as required.
- Ensure accurate and timely data input, maintaining the integrity of patient records.
- Deal with requests for private work or death administration, ensuring prompt and accurate recording of patient information.
- Undertake other general clerical duties as required by the practice.
- Must have a smart appearance and maintain a professional demeanor.
- Must possess excellent written and communication skills, with the ability to interact effectively with patients and staff.
- Must have basic IT skills and be proficient in using practice software and systems.
- Must be a good listener, with the ability to empathize with patients and provide exceptional care.
