Job Description:
HF Trust is seeking a highly motivated and compassionate Learning Disability Care Coordinator to join our team in Edenbridge. As a key member of our team, you will play a vital role in supporting individuals with learning disabilities to access essential services and resources, enabling them to live independently and fulfill their potential.

The ideal candidate will have a strong understanding of disability support principles and practices, as well as excellent organizational and communication skills.

As a Learning Disability Care Coordinator, you will be responsible for coordinating and facilitating access to healthcare services, educational programs, and community activities, as well as providing advocacy and support to service users and their families.

We offer a competitive salary of £12.00 per hour, with opportunities for overtime and bonuses. Our benefits package includes generous holidays, training and development opportunities, and a fully funded Level 2 Diploma in Health & Social Care.

Key Responsibilities:
  • Coordinating and facilitating access to healthcare services and educational programs.
  • Providing advocacy and support to service users and their families.
  • Maintaining accurate records and reporting on progress.
  • Building strong relationships with service users, their families, and other stakeholders.
