Key Responsibilities:
* Develop and implement care plans that meet the needs of service users.
* Lead and manage a team of nurses to ensure the delivery of high-quality care services.
* Evaluate clinical performance, patient outcomes and adherence to care standards.
Requirements:
* Registered Nurse with valid NMC registration.
* Significant experience in homecare, community nursing or complex care settings.
* Strong clinical assessment and care planning skills.
* Deep understanding of safeguarding, risk management and infection control practices.
* Compassionate, patient-centred and dedicated to delivering excellent care.
