We are seeking a highly skilled Pharmaceutical Specialist to join our team, contributing to the delivery of our vision and mission. As a Clinical Medication Optimisation Lead, you will play a key role in developing and implementing strategies to improve prescribing practices and reduce medication errors.

The ideal candidate will have a strong background in pharmacy and experience in medicines optimisation, with a proven track record of improving patient outcomes. You will be a registered pharmacist with the General Pharmaceutical Council and hold a minimum of 3 years post-qualification experience. A strong understanding of clinical governance and quality improvement initiatives is also essential.

About the Role:

  • Provide expert guidance on medicines optimisation and ensure the safe and effective use of medications across the PCN.
  • Develop and implement strategies to improve prescribing practices and reduce medication errors.
  • Work collaboratively with a multidisciplinary team to deliver high-quality patient care and improve health outcomes.
  • Contribute to the development and implementation of clinical governance and quality improvement initiatives.

Requirements:

  • Registered Pharmacist with the General Pharmaceutical Council.
  • Minimum 3 years post-qualification experience in pharmacy.
  • Strong understanding of clinical governance and quality improvement initiatives.
