Palliative Care Specialist Nurse

Job Summary:
We are seeking an experienced and skilled Specialist Palliative Care Nurse to join our team at St Margaret of Scotland Hospice. The successful candidate will work closely with our multi-professional team to provide specialist palliative care to patients living at home with life-threatening illnesses and their carers.
Key Responsibilities:
- Work closely with the multi-professional team to provide specialist palliative care within the Hospice and community setting.
- Participate in daily Ward Rounds, weekly Grand Ward Rounds, and daily Patient handovers within the Specialist Palliative Care Centre St Josephs Ward.
- Establish oneself as a resource, facilitator, and role model for Specialist Palliative Care within the Hospice in-patient and community settings.
- Adopt a multi-professional and collaborative team approach to care assessment and delivery to ensure the needs of community-based patients and families are met.
- Participate in the Hospice Virtual Clinic and/or deputise for the Community Palliative Care Lead as and when required.
- Liaise with the Hospice Social Worker to optimise the psychosocial aspects of the patient and family from a quality-of-life perspective.
- Liaise with the Hospice Therapy Team to optimise the functional aspects of the patient and family from a quality-of-life perspective.
- Support families/carers in bereavement, referring appropriately to child/young person and adult Bereavement Services, should long-term support be required.
Requirements:
The successful candidate will have the skills and experience of working and promoting Palliative Care within a Hospice and/or community or Hospital setting. Competence in the management of symptoms associated with life-threatening illness, effective communication, and interpersonal skills are an essential part of the specialist role.
