Job Title: Specialist Palliative Care Nurse Community

Job Summary:

We are seeking an experienced and skilled Specialist Palliative Care Nurse to join our team at St Margaret of Scotland Hospice. The successful candidate will work closely with our multi-professional team to provide specialist palliative care to patients living at home with life-threatening illnesses and their carers.

Key Responsibilities:

  1. Work closely with the multi-professional team to provide specialist palliative care within the Hospice and community setting.
  2. Participate in daily Ward Rounds, weekly Grand Ward Rounds, and daily Patient handovers within the Specialist Palliative Care Centre St Josephs Ward.
  3. Establish oneself as a resource, facilitator, and role model for Specialist Palliative Care within the Hospice in-patient and community settings.
  4. Adopt a multi-professional and collaborative team approach to care assessment and delivery to ensure the needs of community-based patients and families are met.
  5. Participate in the Hospice Virtual Clinic and/or deputise for the Community Palliative Care Lead as and when required.
  6. Liaise with the Hospice Social Worker to optimise the psychosocial aspects of the patient and family from a quality-of-life perspective.
  7. Liaise with the Hospice Therapy Team to optimise the functional aspects of the patient and family from a quality-of-life perspective.
  8. Support families/carers in bereavement, referring appropriately to child/young person and adult Bereavement Services, should long-term support be required.

Requirements:

The successful candidate will have the skills and experience of working and promoting Palliative Care within a Hospice and/or community or Hospital setting. Competence in the management of symptoms associated with life-threatening illness, effective communication, and interpersonal skills are an essential part of the specialist role.
