">We are seeking an experienced Nurse to join our Intermediate Care Team, based in Kirklees. The successful candidate will work collaboratively with our multi-disciplinary team to provide holistic and patient-centred care to service users.
">Job Responsibilities
">The successful candidate will be responsible for:

- Providing assessment, treatment, and evaluation of clients' needs using a holistic and patient-centred approach.">
- Working collaboratively with our multi-disciplinary team to rehabilitate service users and prevent unnecessary hospital admissions.">
- Supporting and skill-sharing with colleagues within the team and across different sectors.">
