**Responsibilities**:
**Assistant Practitioner (will consider 3-4 development role)**

**Job Location - Kirklees - Intermediate Care Beds**

**Salary: Band 3/4 (£22,816 - £27,596) depending on experience**

**Hours per Week**:37.5 Hours/Week (part-time hours considered)

**Job Type**:Permanent

Within Locala’s Health and Wellbeing Integrated Communities we deliver a range of multidisciplinary community services, including integrated health and social care with our colleagues in Kirklees Council. Locala has been successful in our service improvement work and planning with enhanced service offers providing support to patients, improving their experience, and ensuring excellent continuity of care.

The Assistant practitioner role is a fantastic opportunity to develop your rehabilitation skills. The post will involve therapists e.g. mobility and transfer practice, completion of exercises. The development aspect will focus on Physiotherapy and Occupational Therapy skills and becoming competent with these e.g. Therapy initial assessments, progression of walking aids, keyworker tasks, leading home visits and access visits etc.

The service is based within Ings Grove House, serving the residents of North and South Kirklees, there will also be a need to work within patients home across Kirklees to support our outreach services.

We offer a 7-day service, working 8am until 7pm in a rota pattern providing assessment, treatment and evaluation of clients’ needs using a holistic and patient centred approach. The ultimate aim being to prevent unnecessary future admission to hospital and to ensure the swift appropriate discharge from hospital to offer rehabilitation and enable our clients to return home. Working with initiative is essential, and there will be opportunities to support and skill share with colleagues within the team as well as across the different sectors.

This is an exciting time to be part of a developing, although well-established team, with a number of service transformation projects in progress. Regular clinical and managerial supervision will be received, and the opportunity to develop clinical skills.
