Clinical Case Manager

We are seeking a highly skilled and motivated Clinical Case Manager to join our team at Childrens Complex Care. As a Clinical Case Manager, you will be responsible for managing a caseload of children and young people with complex healthcare needs in the community setting.
Key Responsibilities:- Assess and develop individualized care plans to ensure safe and effective care delivery.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and communication.
- Provide education and training to staff on clinical delegated tasks and ensure competency assessments are completed.
- Carry out staff appraisals and supervisions to ensure high standards of care are maintained.
- Engage with ICBs and external multidisciplinary teams to ensure effective care delivery and collaboration.
- UK registered nurse (RSCN or RGN) with strong clinical, management, and communication skills.
- Experience in managing complex care cases and developing individualized care plans.
- Ability to work effectively in a community setting and collaborate with multidisciplinary teams.
- Strong education and training skills to ensure staff competency and high standards of care.
