Job Title: Clinical Lead Practitioner

We are seeking an experienced Clinical Lead Practitioner to join our team at Kings College Hospital. The successful candidate will be responsible for leading a team of Frailty Practitioners in the delivery of person-centered care, frailty screening, and comprehensive assessment for older people and people with frailty across the Frailty Pathway.

Main Responsibilities:
  • Lead a team of Frailty Practitioners in the delivery of the King's Frailty service, providing day-to-day line management and support.
  • Develop and implement clinical protocols and strategies to enhance care, working closely with the Consultant Team and Dementia and Delirium Specialists.
  • Manage a patient caseload, making and receiving referrals, assessing and facilitating timely discharge for patients, and preventing unnecessary admissions to an acute bed.
  • Coordinate a seamless service through the development of enhanced interdisciplinary team processes and communication.
Requirements:
  • Professional registration as a Nurse, Physiotherapist, or Occupational Therapist.
  • Minimum undergraduate degree relevant to profession.
  • Post-graduate degree relevant to specialist area, or willingness to work towards.
  • Experience in managing a clinical team and relevant, demonstrable experience in the specialty area.

The ideal candidate will have excellent communication skills, both oral and written, and be able to work effectively with all other disciplines within the Trust. In carrying out the duties of the post, the employee is required to work by the policies and procedures of King's College Hospital, including Health and Safety, confidentiality, data protection, and equal opportunities.
