Key Responsibilities
  • Face-to-face work with service users, their families, and external partners
  • Identify whether support packages continue to meet the needs of service users and their families
  • Make recommendations regarding support packages and arrange and amend support
  • Attend multi-disciplinary meetings and present cases
  • Communicate effectively with service users, their families, and internal and external colleagues
  • Write accurate and comprehensive support plans and assessments

This role requires excellent communication skills, both verbal and written, and the ability to work in a person-centred and strength-based way.
