We are seeking a highly skilled and dedicated Physiotherapist to join our team at Practice Plus Group, specializing in Fragility Care. This is an exciting opportunity to make a real difference in the lives of patients in our prisons, working closely with a multidisciplinary team to deliver high-quality physiotherapy support.
Key Responsibilities- Develop and deliver a frailty framework to prevent and manage long-term conditions, including obesity, hypertension, and diabetes.
- Deliver courses and workshops to patients within the prison setting, promoting education and empowerment.
- Exercise critical thinking in safe clinical decision-making, ensuring the best possible outcomes for patients.
- Provide comprehensive care to patients, from initial history taking to diagnosis, treatment, and evaluation of care.
- Collaborate with interdisciplinary teams across sites to meet patient needs, supporting the delivery of the Frailty framework and LTC workstream.
- A competitive annual salary up to £47,000, depending on experience.

Requirements- Registered Physiotherapist (HCPC and MCSP) with a specialist interest in care of the older person, comprehensive geriatric assessment, frailty, dementia, or end-of-life care.
- Strong IT skills and IT literacy, including competence in digital platforms for consultation and Outlook and SystmOne.
- Evidence of relevant experience within physiotherapy and Continued Professional Development (CPD).
- A commitment to maintaining a high standard of clinical practice through self-directed learning, peer supervision, and attendance at training and supervision sessions.
