About the Role

As a highly experienced Paediatric Nurse, you will have a passion for delivering complex care to children and young people. This role involves working closely with a core team of carers, training and supporting them with their clinical skills and activities.

The successful candidate will be responsible for designing and delivering tailored care plans specific to individual clients' needs. You will effectively manage the multi-disciplinary team through strong communication and teamwork.

You will provide ongoing monitoring, support, and assessment of clients' physical and physiological needs within your caseload. Building and maintaining sophisticated relationships with your clients and family members is also an essential part of this role.

This position requires a supportive management style that can bring out the best in others. You will work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care.

Your Key Responsibilities

  • Designing and delivering tailored care plans specific to individual clients' needs
  • Effectively managing the MDT through strong communication and teamwork
  • Providing ongoing monitoring, support, and assessment of clients' physical and physiological needs within your caseload
  • Building and maintaining sophisticated relationships with your clients and family members
  • Working closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care
