About the Role

We are seeking a highly skilled and dedicated Community Mental Health Practitioner to join our team at Midlands Partnership NHS Foundation Trust. As a key member of our CYP crisis and intensive support team, you will play a vital role in providing high-quality clinical services to children and their families.

Key Responsibilities
  • Deliver a range of interventions, including pharmacological and psychologically informed approaches, to support children and families experiencing mental health-related difficulties.
  • Assess patient needs, develop, implement, and evaluate care plans, ensuring effective coordination with multidisciplinary teams.
  • Participate in the development of the service, ensuring clinical governance processes are actively implemented at the team level.
  • Collaborate with senior clinical and operational teams to achieve the aims and objectives of the team.
  • Work within relevant professional codes of conduct and guidelines.
Requirements
  • Registered Mental Health Practitioner/Nurse on NMC or Social Work England Register.
  • Conforms to NMC/Social work England requirements.
  • Experience of working with people experiencing mental health-related difficulties.
  • Ability to assess, plan, coordinate, and manage people with mental health problems in a community setting.
