Job Summary

We are seeking a highly skilled and compassionate Assistant Reablement Coordinator to join our team in Cheshire West and Chester. As an essential member of our reablement team, you will play a vital role in supporting individuals to live independently in their communities.

About the Role

The successful candidate will be responsible for working with service users in their own homes, providing support and guidance to help them regain their independence and confidence. You will work closely with the reablement team to deliver a high-quality service, promoting sustainable living and community integration.

Key Responsibilities
  • Supporting service users in their daily lives, including managing daily living skills and setting achievable goals
  • Working collaboratively with the reablement team to deliver a high-quality service
  • Ensuring that service user journeys are well-coordinated and compliant with organisational policies and regulations
Requirements

To be successful in this role, you will need:

  • Experience of delivering frontline care, including reablement
  • Knowledge of internal policies and procedures relating to social care, including Care Quality Commission standards and regulations
  • Understanding of related legislation and manual handling practices
  • Developed IT skills, including Microsoft Office
