About This Opportunity

Tripod Partners is seeking a Comprehensive Care Coordinator to join our dynamic team. As a key member of our team, you will be responsible for undertaking comprehensive assessments of individuals with learning disabilities, identifying their needs and strengths.

Job Summary

This role involves developing and implementing person-centered care plans that promote independence and well-being, collaborating with multidisciplinary teams, and advocating for the rights and needs of individuals with learning disabilities.

Responsibilities and Requirements
  • Undertake comprehensive assessments of individuals with learning disabilities, identifying their needs and strengths.
  • Develop and implement person-centered care plans that promote independence and well-being.
  • Collaborate with multidisciplinary teams, including healthcare professionals, housing providers, and other relevant agencies.
  • A Social Work qualification (Degree or Diploma level).
  • Registration with Social Work England.
  • Proven experience working with individuals with learning disabilities.
  • Strong assessment and care planning skills.
