About This Role
The successful candidate will work as a Diabetes Specialist Nurse within primary care settings, utilizing their expertise to optimize patient care. A strong focus on education and support is essential for this position.
Responsibilities
- Assess and manage patients with Type 2 diabetes, optimizing their oral therapy
- Collaborate with multidisciplinary teams to provide comprehensive patient care
- Develop and implement personalized care plans
- Provide education and support to patients, families, and caregivers
Requirements
- Registration with the NMC and current practicing certificate
- Minimum of 3 years' experience in primary care settings
- Excellent communication and interpersonal skills
- Familiarity with IT systems, including primary care software
- Ability to drive and access various locations
