About This Role

The successful candidate will work as a Diabetes Specialist Nurse within primary care settings, utilizing their expertise to optimize patient care. A strong focus on education and support is essential for this position.

Responsibilities

  • Assess and manage patients with Type 2 diabetes, optimizing their oral therapy
  • Collaborate with multidisciplinary teams to provide comprehensive patient care
  • Develop and implement personalized care plans
  • Provide education and support to patients, families, and caregivers

Requirements

  • Registration with the NMC and current practicing certificate
  • Minimum of 3 years' experience in primary care settings
  • Excellent communication and interpersonal skills
  • Familiarity with IT systems, including primary care software
  • Ability to drive and access various locations
