Job Title: Advanced Practitioner - Adult Social Care

We are seeking a highly skilled and experienced Advanced Practitioner to join our Adult Social Care team. As a key member of our team, you will be responsible for working across Older People, Physical and Learning Disabilities, Hospital Discharge referrals via Home First, and all age Carers assessments.

Key Responsibilities:
  • Supervise and support the professional development of social workers and social care assessors.
  • Organize daily duties while adhering to SCC policy and procedure to meet service demands.
  • Ensure the quality assurance of statutory documents and promote the ethos of Just Enough Support.
  • Manage a healthy work-life balance and ensure flexibility.
  • Motivate staff to deliver objectives around adult engagement.
  • Promote and support the identification of actual or potential safeguarding issues and ensure prompt action.
  • Co-work cases with less experienced team members, taking responsibility for sharing and developing their learning.
  • Participate in the inter-agency Best Interest Assessor rota for adults.
  • Participate in the inter-agency AMHP rota.
  • Have knowledge and experience of working within Adult Social Care.
  • Have knowledge of mental health legislation, including section 117.
  • Understand the principles of different funding streams available to support adults.

We require you to be flexible to work at any other location across the service, as and when required, and be available to work out of hours, evenings, and weekends when required.
