We are seeking a highly skilled and dedicated Mental Health Practitioner to join our team in Primary Care. As a key member of our multidisciplinary team, you will play a vital role in improving access to mental health care for individuals with mild to moderate or stable severe mental health problems.
Main Responsibilities- Deliver holistic assessments and evidence-based care to clients within the context of a multi-disciplinary, multi-agency primary mental health service.
- Work collaboratively with GPs and other primary care providers to provide support and advice.
- Develop effective relationships between primary care and local communities, and other services in the voluntary and statutory sector.
- Provision of information, CCBT, and self-help for people with common mental health disorders.
- Mental health promotion and the delivery of brief, short-term, and low-intensity psychological interventions.
- Registered Nurse (Mental Health), Social Worker, or Occupational Therapist.
- Evidence of post-registration continuing professional development activity.
- Evidence of achievement in clinical practice within the area of specialty, e.g. primary care, community mental health, Older adults, CAMHs.
