Role Overview

Each day at Cornerstone Community Care is distinct and rewarding. In your role as a Community Care Practitioner, you will deliver individualized support to clients, assisting them with daily activities, appointments, personal hygiene, and medication management. You will also engage in various activities such as sports, fitness, and social events to enhance the lives of those we serve.
