About the Role
We are seeking a motivated and caring individual to join our team as a Support Practitioner. As a key member of our team, you will provide tailored and person-centred support to individuals, enabling them to live a fulfilling and valued life.
Key Responsibilities
- Support individuals with daily personal needs, including meal planning, cooking, and housework.
- Assist with appointments and prompting with personal care and medication.
- Encourage participation in hobbies and interests, such as the gym and football.
- Participate in the planning and delivery of person-centred care, promoting the views and wishes of the individuals we support.
- Contribute to the development of a positive and supportive team environment.
About You
To be successful in this role, you will possess excellent communication skills, a caring attitude, and a positive approach to learning. You will be able to think outside the box and be confident in the use of technology. We welcome applications from individuals with transferable skills and a willingness to learn.
