Job Title: Social Worker - Hospital Discharge Team

We are seeking a skilled Social Worker to join our Hospital Discharge Team in Hounslow. As a key member of our team, you will be responsible for ensuring the safe and timely discharge of service users from hospital settings.

Key Responsibilities:
  • Develop and implement care plans for vulnerable adults in hospital settings
  • Conduct assessments and report writing to inform care decisions
  • Liaise with other professional bodies to ensure seamless care transitions
  • Manage complex cases and provide support to service users and their families
  • Collaborate with healthcare professionals to ensure safe and timely discharges
Requirements:
  • Post-qualifying experience as a Senior Social Worker in the UK Social Care industry
  • HCPC registration and enhanced DBS/CRB clearance
  • Eligibility to work in the UK
