Job Summary
We are seeking a highly skilled and compassionate Care Navigator to join our team. As a Care Navigator, you will play a vital role in ensuring that patients receive the best possible care and support.
Main Responsibilities
- Process all general enquiries, explain procedures, and make new and follow-up appointments in a courteous and efficient manner.
- Process appointment requests for today/future appointments from patients by telephone and in person, and actively process home visits/requests via patient triage.
- Ensure callers are directed to the appropriate healthcare professional, providing accurate and timely information.
- Maintain the reception area, notice-boards, and leaflet dispensers in a tidy and organized state, free from obstructions and clutter.
- Explain practice arrangements and formal requirements to new patients and those seeking temporary cover, ensuring procedures are completed accurately and efficiently.
- Receive and make telephone calls as required, diverting calls and taking messages with accuracy and promptness.
- Enter requests for home visits into the visit book, carefully recording all relevant details and referring to the duty doctor where necessary.

- In the course of seeking treatment, patients entrust us with sensitive information in relation to their health and other matters. As a Care Navigator, you will respect patients' privacy and act accordingly.
- You will have access to confidential information relating to patients, carers, practice staff, and other healthcare workers. You must regard all such information as strictly confidential.
- Information may only be divulged to authorized persons in accordance with practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.
Person Specification
Qualifications
- Experience within a healthcare environment or similar.
- Experience with front-facing the public.
Desirable
- Experience in call handling skills.
