About the Role

We are seeking a highly skilled and compassionate Adult Mental Health Practitioner to join our team at Berkshire Healthcare NHS Foundation Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care to older adults experiencing functional mental health difficulties and dementia in the community setting.

You will be based at Hillcroft House, located on the West Berkshire Community Hospital site, and will work closely with our team to develop and implement effective care plans for our patients. Your expertise and experience will be invaluable in ensuring that our patients receive the best possible care and support.

Key Responsibilities:

  • Develop and implement care plans in collaboration with patients, families, and carers
  • Monitor and evaluate care plans, making adjustments as necessary
  • Develop and maintain effective communication with GPs, local authorities, and voluntary organizations
  • Participate in service development to enhance nursing contributions and quality of care
  • Ensure adherence to Trust policies
  • Develop specialist knowledge and skills in older adult mental health

Requirements:

  • Dual qualified RMN and RGN
  • Completion of postgraduate course relating to older adults
  • 2+ years of experience working with older people with mental health needs in a range of clinical settings
  • Advanced interpersonal and communication skills
  • Leadership abilities
  • Ability to work autonomously and as part of a team
