About the Role

We are seeking a highly skilled and compassionate Adult Mental Health Practitioner to join our team at Berkshire Healthcare NHS Foundation Trust. As a key member of our multidisciplinary team, you will work closely with patients, families, and carers to provide high-quality care and support.

The successful candidate will have a strong background in mental health nursing, with experience working with older adults in a community setting. You will be responsible for developing and implementing care plans, monitoring patient progress, and collaborating with other healthcare professionals to ensure seamless care.

Key Responsibilities
  • Develop and implement care plans in collaboration with patients, families, and carers
  • Monitor patient progress and adjust care plans as needed
  • Collaborate with other healthcare professionals to ensure seamless care
  • Participate in service development to enhance nursing contribution and quality of care
  • Ensure adherence to Trust policies and procedures

Person Specification
  • Dual qualified RMN and RGN
  • Completion of Post Graduate Course relating to Older Adults
  • Experience in multidisciplinary working
  • 2 years+ experience of working with Older People with mental health needs in a range of clinical settings
