  • Care Coordination
    • Coordinate and manage the delivery of care and support services to individuals with mental health issues and their carers.
    • Ensure that care plans are implemented and reviewed regularly to meet the changing needs of individuals.
  • Advocacy and Support
    • Provide advocacy and support to individuals with mental health issues and their carers to ensure their rights and interests are protected.
    • Work with individuals and their carers to develop coping strategies and promote independence.
  • Collaboration and Communication
    • Develop and maintain effective relationships with other professionals, including healthcare providers, social services, and voluntary organizations.

Requirements
  • Qualifications
    • Degree in Social Work or a related field.
    • Post-qualifying experience in mental health social work.

    • Strong communication and interpersonal skills.
    • Knowledge of mental health legislation and policy.

What We Offer
