Adult Mental Health Social Worker

Main Purpose of Post:
To provide comprehensive mental health support to individuals and their carers, ensuring their well-being and promoting social inclusion.
Liaise with professionals to ensure seamless care pathways, safeguarding and promoting independence.
Promote joint working to deliver person-centred outcomes for individuals with mental health issues.
Undertake safeguarding investigations as required.
Assist the Local Authority in meeting statutory obligations under the Mental Health Measure and the Social Services and Wellbeing (Wales) Act 2014.
Deliver on the Local Authorities statutory requirements within the MHA 1983 as an AMHP.
Key Responsibilities:
- Assess the needs of individuals with mental health issues and their carers holistically, support negotiated, monitored and reviewed within the first six weeks.
- Maintain an allocated caseload.
- Assess risks to individuals with mental health issues and others, promoting independence and ensuring necessary safeguards are in place.
- Make recommendations about allocating scarce resources.
- Promote equal opportunity and respect for diversity and difference.
- Promote social inclusion of isolated and vulnerable people.
- Investigate abuse of vulnerable adults.
- Work collaboratively with other agencies.
- Accountable for actions through effective, accurate recording of decisions in line with policy and legislation.
- Safeguard the welfare of a vulnerable person at immediate risk of harm.
- Engage in direct work with individuals with mental health issues and carers to carry out assessments.
- Enhance problem-solving capacities in individuals with mental health issues and carers to support maximum independence and choice.
- Consider options to best meet assessed eligible needs and assist in making informed decisions.
- Undertake the role of an AMHP.
- Conduct Mental Capacity Act assessments and Best Interests decisions alongside advocates where appropriate.
- Adopt a creative and holistic approach while exploring options.
- Maximise financial and material resources available to service users from all possible sources.
- Record unmet need.
- Collate the findings of the assessment and complete assessment documentation in accordance with statutory guidance, legislation and departmental procedures and timescales.
- Prepare court and other specialist reports in the required format.
- Ensure case file recording and all relevant documentation is completed in accordance with departmental policy and procedures.
- Manage conflict effectively within individual, family and professional situations.
- Use professional assertiveness to justify decisions and uphold professional social work practice.
- Challenge judgements and decisions of others where there is evidence that the needs of those being worked with are not being met.
- Convene and/or attend interagency meetings/ liaison e.g. case conferences.
- Obtain managerial/panel agreement for personal plan of care delivery.
- Record and disseminate care and treatment plans in accordance with statutory requirements, legislation and departmental procedures.
- Contribute to shaping services both directly and indirectly.
- Understand budget and resource implications in constructing high-quality and value-for-money care provision.
- Encourage service users to consider positive risk-taking to move forward with their lives.
- Delegate monitoring tasks (if appropriate) to another individual e.g. reviewing officer, care provider following six-week review.
- Arrange and co-ordinate review of the Care and Treatment Plan for ongoing caseload.
- Engage directly with individuals with mental health issues, families, carers, in the review of the Care and Treatment Plan and agree changes where necessary.
- Negotiate and agree changes to plan with providers and other agencies involved.
- Ensure individuals with mental health are aware of complaints procedures and advocacy services.
- Contribute to the professional development of others by acting as a mentor for newly qualified practitioners undertaking professional training following first year in practice.
- Timely administration of all aspects of caseload.
- Prepare and engage in professional supervision with Line Manager.
- Establish and maintain good working relationships with colleagues.
- Identify and address areas for professional development and training.
- Comply with Welsh language requirements of the position.
- Adhere to health and safety advice and guidance.
- Challenge discrimination and abide by the principles and practice as laid down in the Councils Equal Opportunities Policy.

- This position has a requirement for an Enhanced DBS and ISA registration as it is classed as Regulated Activity under the Vetting and Barring Guidance, 2009.
- This post is exempt from the Rehabilitation of Offenders Act.
- Demonstrate Continual Professional Development including meeting the requirements of Social Care Wales for continued registration as a Social Worker.
- Share skills, knowledge and learning with colleagues in other specialisms.
- Meet the requirements of the national occupational standards in social work and adhere to the code of practice for social workers.
- Undertake post-qualifying training which include practice assessor module, best interest assessor, AMHP or post-qualifying assessing.
- Seek feedback on performance from service users, carers, colleagues, managers and mentors to reflect on practice.
- Prepare for and engage in practice observations by Line Manager or Case Work Manager.
- Contribute to team/peer learning programmes.
- Strive to be an effective team player.
- Record and agree with Manager examples of progress and learning to meet re-registration requirements.
