We are seeking a highly skilled and motivated Health Visitor to join our team at Sandwell and West Birmingham NHS Trust. As a Health Visitor, you will play a vital role in providing high-quality care to children and families in our community.
Key Responsibilities- Conduct comprehensive assessments of children and their carers to identify health and wellbeing needs
- Develop and implement personalized care plans to promote healthy lifestyles and prevent illness
- Work in partnership with other agencies and professionals to ensure seamless care delivery
- Provide education and support to families on healthy eating, physical activity, and other health-related topics
- Collaborate with community groups and organizations to promote health and wellbeing initiatives
- Registered General Nurse with a Degree or Diploma in Health Visiting
- Qualified Nurse Prescriber
- Experience of working in partnership with other agencies and professionals
- Excellent communication and interpersonal skills
- Ability to work flexibly and adapt to changing circumstances
