We are seeking a highly skilled and motivated Health Visitor to join our team at Sandwell and West Birmingham NHS Trust. As a Health Visitor, you will play a vital role in providing high-quality care and support to children and families in our community.
Key Responsibilities- Conduct comprehensive assessments of children and their carers to identify their needs and develop effective care plans.
- Provide evidence-based care and interventions to promote the health and well-being of children and families.
- Work in partnership with other agencies and professionals to ensure seamless care and support.
- Develop and maintain strong relationships with families and communities to promote health and well-being.
- Contribute to the development and implementation of health promotion programs and initiatives.
- Registered General Nurse with a degree or diploma in Health Visiting.
- Qualified Nurse Prescriber.
- Experience of working in partnership with other agencies and professionals.
- Excellent communication and interpersonal skills.
- Ability to work flexibly and adapt to changing circumstances.
