Palliative Care Nurse

We are seeking a highly skilled and compassionate Registered Nurse to join our team in Devon, providing specialist palliative care to individuals with terminal illnesses and their families.
Key Responsibilities- Deliver high-quality, person-centered care to patients in their own homes, focusing on symptom management, medication administration, and emotional support.
- Collaborate with multidisciplinary teams to coordinate care, ensuring seamless transitions and optimal patient outcomes.
- Develop and implement individualized care plans, prioritizing patient needs and preferences.
- Provide clinical leadership and support to our team of Health Care Assistants, promoting a culture of continuous quality improvement and professional development.
- Contribute to the development of our service, identifying opportunities for innovation and improvement.
