Position Overview
Are you an experienced Clinical Nurse Practitioner? Leverage your medical knowledge to contribute positively in our healthcare setting.
As a vital member of a collaborative healthcare team, you will evaluate and manage patients presenting with intricate and/or long-term health conditions and/or injuries, ensuring accurate clinical assessments, treatment plans, and appropriate patient discharge.
Key Responsibilities
Drawing on your triage experience from urgent care environments, you will engage with patients presenting with complex and/or chronic health issues, having been referred by their primary care provider. You will independently conduct clinical evaluations to establish diagnoses, treatment options, and discharge plans.

Job Duties

- Conduct independent clinical assessments, diagnose, treat, and discharge patients with a variety of minor health issues and injuries, including complex cases
- Collaborate as a key member of the healthcare team to provide effective in-person evaluations to ensure patients are directed to the appropriate clinician based on their clinical needs
- Stream and prioritize patients presenting at the facility
- Facilitate patient access within operational hours and ensure a clear care pathway
- Provide health education and advice as necessary
- Empower patients to engage in self-care through effective communication of health information
- Recognize critically ill patients and respond appropriately, coordinating with other clinical staff as needed
- Identify safeguarding issues and make necessary referrals in accordance with established protocols
- Adapt to a dynamic work environment, responding positively to new challenges and demonstrating flexibility
- Promote and support evidence-based practice and quality improvement initiatives
- Direct patients to the appropriate healthcare professional in a timely manner
- Assess and prioritize patients based on their clinical urgency while ensuring safety
- Perform ECGs, assist with patient care tasks, and support colleagues as required
- Collaborate with team members to ensure timely patient care delivery

Essential Qualifications
- Registered Nurse with relevant qualifications
- Experience in triage and patient assessment
- Proven track record of working with diverse patient populations requiring assessment and treatment (minimum of one year experience)
- Advanced Clinical Practice qualification or equivalent credentialing
