Senior Community Nursing Manager

We are seeking a dedicated Senior Community Nursing Manager to become part of a reputable and innovative organization specializing in comprehensive care solutions for adults in the community. This role is crucial as the company expands its services and enhances its care offerings.
The ideal candidate will be responsible for delivering in-person support to adult clients and their families, while also developing customized care strategies tailored to individual needs. You will collaborate closely with a dedicated team of caregivers, providing training and guidance to enhance their clinical competencies and daily activities.
Qualifications for the Senior Community Nursing Manager:
- Registered General Nurse (RGN) with a valid NMC PIN
- Empathetic leadership style that fosters team development
- Recent experience in tracheostomy management and ventilator care within the last two years
- Exceptional verbal and written communication skills for effective interaction with patients, families, and multidisciplinary teams
- Appropriate for Band 5 / Band 6 Nurses with proven leadership and management experience of Healthcare Assistants (HCAs)
- Full UK Driving Licence is preferred but not mandatory
- Develop and implement personalized care plans tailored to each client's requirements
- Effectively coordinate the multidisciplinary team through strong communication and collaboration
- Provide continuous monitoring, support, and assessment of clients' physical and emotional needs within your caseload
- Administer nursing interventions, including medication management
- Establish and nurture meaningful relationships with clients' families
- Work in partnership with community teams, Continuing Healthcare (CHC) nurses, and Clinical Commissioning Group (CCG) representatives to create and sustain customized care packages
- Competitive salary up to £45,000 per annum, depending on experience
- One day of remote work per week with four days in the community
- Standard working hours from Monday to Friday (9 AM - 5 PM)
- Generous holiday allowance of 33 days
