Job Title: Band 6 Professional Development Nurse

We are seeking a highly skilled and motivated Registered Nurse to join our team as a Band 6 Professional Development Nurse. This is an exciting opportunity to work in a dynamic and supportive environment, delivering high-quality nursing care to patients in their own homes.

About the Role

As a Band 6 Professional Development Nurse, you will be responsible for assessing, implementing, and evaluating person-centered care for a caseload of patients. You will work under the direction of a District Nurse, using your clinical skills and knowledge to provide care and treatment to patients with a range of needs, including those with long-term conditions, complex care needs, and end-of-life care.

You will be required to work autonomously, using your initiative and problem-solving skills to prioritize visits and manage your workload. You will also be expected to work collaboratively with other healthcare professionals, including GPs, Practice Nurses, and social care teams, to ensure seamless care and support for patients.

Key Responsibilities
  • Assess, implement, and evaluate person-centered care for a caseload of patients
  • Work under the direction of a District Nurse, using clinical skills and knowledge to provide care and treatment to patients
  • Use electronic patient record systems, including EMIS, to record and manage patient information
  • Work collaboratively with other healthcare professionals to ensure seamless care and support for patients
  • Develop and maintain relationships with patients, families, and carers to ensure high-quality care and support
Requirements
  • Registered Level 1 Nurse with current Nursing and Midwifery Council registration
  • Educated to diploma or level 6/degree level in a relevant area
  • Supervisor/Mentorship qualification (within the first 2 years of post)
  • Evidence of continuing professional development
  • Area of special interest in the management and treatment of long-term conditions and willingness to undertake additional education in this area
