About the Role

We are seeking an experienced Mental Health Community Nurse to join our Crisis Function team. As a key member of the team, you will work with service users at various stages of their mental health journey, providing urgent assessments and robust home treatment options as an alternative to hospital admission.

Main Responsibilities
  • Provide high-quality, person-centered care to service users, ensuring their needs are met and their care plans are updated regularly.
  • Work collaboratively with the multi-disciplinary team to provide safe and effective treatment in the community, facilitating early discharges from acute inpatient units.
  • Develop and maintain positive relationships with service users, their carers, and other agencies, ensuring effective communication and collaboration.
  • Assess and manage risk in the acute care pathway, enabling and empowering individuals to recover and manage their mental health.
  • Deliver evidence-based care, adhering to Trust policies and procedures, and maintaining high standards of record keeping and documentation.
